Why conduct an Incident Investigation
Investigating incidents and near misses is an important part of a Health and Safety Management System and key for legislative compliance.Our team conducts investigations to identify both the factors that contribute to an incident and the root causes behind those factors. Using Root Cause Analysis, we systematically review the facts that contribute to an incident in order to move focus from the symptom or immediate cause(s), to the underlying or root cause.Through the investigation process, the contributing factors and root causes are identified that if corrected, would have prevented the incident from occurring or would have significantly mitigated its consequences.When identifying causal factors / root causes we look for where ‘the system failed the worker’ not only for those where ‘the worker failed the system’.Qualifications
Our team has the proper combination of experience, knowledge, and education to properly investigate incidents, accidents, and near misses.The Risk Refined team is comprised of experienced Canadian Registered Safety Professionals (CRSP) who have assisted many clients in completing the following types of investigations:
- Equipment / Property Damage.
- Injury Events (Fatality, Lost Time Injury, Medical Aid, Modified Work, First Aid).
- Potentially Serious Incident (PSI).
- Refusal to Do Dangerous Work.
- Spill – Release.Our team has received formal TapRooT Advanced Root Cause Analysis Team Leader training, which has been indispensable in order to effectively investigate and document accidents, incidents, and near misses.
